Applications stating income is zero must provide written statement verifying how they pay their bills. 2. Information on the date and place of current or last employment for all household members. Any member 18 or older without a job must provide verification of unemployment. 3. Proof of child support being received. 4. Proof of utility assistance if applicant received subsidized housing. 5. A copy of an energy bill or receipt from energy supplier.
